Pea Ridge elementary kindergarten registration Pea Ridge Elementary Kindergarten registration for the 2008 / 09 school year for kindergarten students will be held 8: 30 a.m. to 2 p.m. each day April 28 through May 2 and May 5 through 9. Students entering kindergarten for the 2008 / 09 school year must be 5 years old on or before Sept. 15, 2008, according to Arkansas state law. A student will not be enrolled until all of the following documents have been presented to the school Proof of Physical (filled out by physician ) or signed exemption form (available at registration ).

Proof of birth date: One of the following documents may be used to indicate that child's age: birth certificate, statement by local registrar or county recorder certifying the child's date of birth, attested baptismal certificate, passport, affidavit of date and place of birth by child's parent or guardian, previous school records or U. S. military identification.

Updated Immunization Record: The immunization requirement for kindergarten enrollment are: At least three doses OPV or IPV - the last dose administered after child's 4 th birthday; at least five doses DTP, DT or DTaP - the last dose after child's 4 th birthday; two MMR - the first dose administered after child's 1 st birthday; three Hepatitis B; one Varicella (chicken pox ) - administered after child's 1 st birthday with a second dose recommended before entering kindergarten. (If the child has had the disease, a signed statement to that effect must be presented to the school. )

• Social Security card Proof of residency in Pea Ridge School District: A current utility bill (water, electric or Arkansas Western Gas accepted ) or real estate tax receipt showing parents' names and 911 address. The following will not be accepted: P. O. box number, personal property receipt or a utility bill that is past due or subject to disconnect. Scholar support needed The Pea Ridge Community Scholarship Foundation, Inc. is a non-profit, tax exempt, organization that provides way to make donations for scholarships to allow students graduating from the Pea Ridge School District to earn a college degree.
